Submit Your Nomination

To submit a nomination for consideration, please follow the following steps.

Step 1: Review the Hall of Fame By-laws for nominating an individual, coach, team, or friend of sports.

Step 2: Open the nomination application PDF form below and print out form.

Step 3: Fill out the form in its entirety.

Step 4: Mail forms to Quincy Blue Devils Sports Hall of Fame, 1535 State Street, Quincy, IL  62301 or fax form to 217-222-1108.
